STEM Kids NC, Inc dba After School Business (ASB) 
Assumption of Risk, Minor Photo Release, Waiver of Liability and Indemnification Agreement

Nature of the Activity:
STEM Kids NC, INC dba After School Brilliance (hereafter referred to as ASB) is an after-school enrichment and day camp which offers clients the opportunity to participate in a number of science, technology, robotics, programming, artistic and entrepreneurship activities.

Assumption Of Inherent Risks:
I ___________________ [parent/legal guardian] have read the above paragraphs and understand that participating in activities facilitated by ASB contain inherent risks which vary with the activity involved. I understand the demands of those activities relative to my child’s physical condition and skill level. I hereby assert that my participation is voluntary and I _________________ [parent/ legal guardian] knowingly assume all inherent risks on behalf of my child.
 

Minor Photo Release:
I___________________ [parent/legal guardian] of ________________________ [Child] grant ASB my permission to use the photographs described as photos/videos taken while ________________________ [child] is attending a ASB activity (after-school/camp/party) for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, conferences and web content.

Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Finally, I understand that in the unlikely event of injury,  I will not hold After School Brilliance liable.
